I am aware that my consent can be withdrawn at any time, resulting in the removal of my e-mail address and name from the distribution list of the „Newsletter” service.

I hereby consent to the company Cezos spółka komandytowa sp. k. to use end-user devices and automatic calling systems for direct marketing as part of the „Newsletter” service (pursuant to art. 172 section 1 of the Polish act of July 16th, 2004, Telecommunications law).
